Assessment Tables/Grids for Family Practices & Assessors

The Quality in Family Practice Assessment Tables/Grids for Family Practices & Assessors contain tables designed to assist the practices and the assessors to conduct the assessment of the family practice. The practices will complete the tables as part of their self-assessment and to demonstrate what they have accomplished. The practices will also be able to measure their improvement over time. The Assessment Tables/Grids contain all of the indicators and criteria found in the Assessment Tool. The tables are designed to rank each criterion on a Likert scale from 1 (Not met) to 5 (Fully met). The practice can supply additional evidence to substantiate its adherence to each criterion, and what systems are in place to ensure consistent adherence to those criteria. This evidence might be in a number of forms such as a descriptive report, a summary of an audit, a patient questionnaire, etc. If the practice does not have a system in place to ensure the criterion will be consistently maintained, the practice should consider what action is needed to rectify this. It is expected that most family practices will be unable to fully meet many of the “desirable” criteria on the preliminary assessment. The focus will be a continuous quality improvement (CQI) process. The practice must demonstrate what steps are being taken and what systems and procedures are being implemented in order to demonstrate quality improvement.

The Practice Summary is the short reports that the family practice provides to the assessors to describe the practice, providers, staff, and practice population it serves. In addition, there are short reports on the services that the practice provides.
